Good practice at end of life for social care practitioners: Practice Tool (2017)
Introduction
This resource explores how social care practitioners can better support people who are dying or bereaved. It is aimed at practitioners such as social workers and those who support them, for example supervisors, managers, and learning and development staff.
The resource looks at:
- what people who are dying or bereaved want and need
- the context and evidence around providing better support
- the role of social care and social work in supporting people
- how practitioners can increase their confidence and capabilities.
The resource includes four practical tools to help with:
- person-centred care
- person-centred planning
- coordinating care
- emotional support for practitioners.
The tool includes quotes from people with lived experience which are taken from What Matters most: Talking with people who are nearing the end of their lives (Gosling and Fox, 2016).
